A useful log connects a reading to its equipment, time, operator and follow-up. It should make an exception easier to investigate, not simply produce another completed form.

Recording is different from continuous monitoring
A manually entered reading describes a measurement taken at a particular time. It cannot establish what happened between readings. Replacing a paper log with software does not create a continuous sensor feed, overnight alarm coverage or a response team.
The MOH-hosted CBAHI CLBB document includes continuous monitoring of critical supply and reagent storage conditions. Confirm the current requirements for your facility and the appropriate monitoring arrangement. This template is not a substitute for that arrangement.
Design the record around the next review
- Identify the storage unit and the measurement device or source.
- Record the observation date, time and time zone, measured value and unit.
- Preserve the limits that applied when the observation was recorded.
- Identify the operator, any exception and a reference to the investigation.
- Record who reviewed it and where the follow-up decision is documented.
The downloadable template includes these organizational fields. It does not determine a safe storage range or automatically assess material suitability. Use manufacturer instructions, approved procedures and qualified review for those decisions.
Make an exception visible without inventing its resolution
For a hypothetical out-of-range observation, the record should distinguish detection, escalation, assessment and closure. “Supervisor informed” is an action; it does not establish that the affected materials are acceptable for use.
Follow your laboratory’s procedure for escalation and handling affected materials. Record the facts available, the decision owner and the supporting reference. Do not fill a missing reading with an estimated value to make the sheet appear complete.
Preserve context when settings change
A later change to a configured range should not silently make an earlier exception disappear. When evaluating a digital system, ask how it preserves historical limits, corrections, operator identity and timestamps. Ask whether the export includes the entire selected period and whether failed data retrieval is visible.
